    You will be required to attest to the qualifications and acknowledgement in front of a filing officer when you sign your documents in office.

  • Salt Lake County Auditor

    Utah Code §17-66-201, §17-69-202, §20A-9-201

    • U.S. Citizen.
    • Registered voter in Salt Lake County.
    • Resident of Salt Lake County for at least one year prior to the date of the election.
    • Shall have one or more of the following professional certifications active and in good standing:
      • Certified public accountant; certified internal auditor; certified fraud examiner; certified management accountant; or certified information systems auditor.
  • Salt Lake County Council

    Utah Code §17-66-201, §17-64-201, §20A-9-201

    • U.S. Citizen.
    • Registered voter in Salt Lake County.
    • Registered voter for at least one year prior to the date of the election.
    • Resident of Salt Lake County for at least one year prior to the date of the election.

    If filing for a Council District:

    • Registered voter in the district.
    • Resident of the district for at least one year prior to the date of the election.
  • Salt Lake County Clerk

    Utah Code §17-66-201, §20A-9-201

    • U.S. Citizen.
    • Registered voter in Salt Lake County.
    • Resident of Salt Lake County for at least one year prior to the date of the election.
  • Salt Lake County District Attorney

    Utah Code §17-68-202, §20A-9-201

    • U.S. Citizen.
    • Registered voter in Salt Lake County.
    • Resident of Salt Lake County for at least one year prior to the date of the election.
    • An attorney licensed to practice law in Utah who is an active member in good standing of the Utah State Bar.
    • Shall submit either:
      • A letter from the Utah Supreme Court, affirming that the individual is an attorney in good standing;
      • Proof of the individual’s application with the Utah State Bar, with an affidavit describing the status of the individual’s application; or
      • An affidavit describing how the individual intends to comply with the requirements for office of county attorney described in Subsection (3)(b)(i), prior to taking office.
    • Shall provide the individual's license number with:
      • Either the Utah State Bar, if the individual is a member,
      • Or any other State Bar Association, if the individual is a member.
  • Salt Lake County Sheriff

    Utah Code §17-66-201, §17-72-201, §20A-9-201

    • U.S. Citizen.
    • Registered voter in Salt Lake County.
    • Resident of Salt Lake County for at least one year prior to the date of the election.
    • Must provide a certificate from the Peace Officer Standards and Training Division stating that the candidate has:
      • Successfully met the standards and training requirements established for law enforcement officers under Title 53, Chapter 6, Part 2, Peace Officer Training and Certification Act;
      • or has met the waiver requirements in Section 53-6-206; and
    • Qualified to be certified as a law enforcement officer as defined in Section 53-13-103.
    • At the time of taking office must be qualified to be certified as:
      • law enforcement officer, as defined in Section 53-13-103; and
      • a correctional officer, as defined in Section 53-13-104; or
      • a correctional facility manager by having successfully completed a correctional facility management course that is offered by a certified academy in both an online web-based format and in a classroom format and that is approved by the Peace Officer Standards and Training Division created in Section 53-6-106.
  • Local School Board Member

    Utah Code §20A-9-201, §20A-14-202

    • U.S. Citizen.
    • Resident of the local school board district in which the person is seeking election for at least one year as of the date of election.
    • Registered voter of the school representative district from which elected.
    • Non-partisan.
  • Affidavit

    Please review the following, you will be required to attest to this statement when you complete and sign your forms in the Clerk's Office.
  • I will meet the qualifications to hold the office, both legally and constitutionally, if selected.

    I will not knowingly violate any law governing campaigns and elections; I will file all campaign financial disclosure reports as required by law; and I understand that failure to do so will result in my disqualification as a candidate for this office and removal of my name from the ballot.

  • Declaration of Candidacy Acknowledgment

  • In accordance with Utah Code §20A-9-201, §20A-6-109, §63G-2-305(52), §20A-9-405, §20A-9-502, and §20A-9-408:

     

    The filing officer read the constitutional and statutory requirements to me, and I meet those requirements.
     

    I have completed my Conflict of Interest Disclosure Form and it is updated and accurate as of the date of filing this Declaration of Candidacy.
     

    I understand that an individual who holds a municipal elected office may not, at the same time, hold a county elected office.
     

    I understand that an individual may only appear on an official ballot as a candidate for one office and one political party.
      

    I understand that my name will appear on the ballot as it is written on the Declaration of Candidacy, and that I may not make any amendments or modifications after the applicable deadline:

    • For candidates filing with a Qualified Political Party: January 8, 2026 at 5:00 p.m.
    • For candidates filing with a Registered Political Party: April 27, 2026 at 5:00 p.m.
    • For candidates filing without a party (unaffiliated): June 15, 2026 at 5:00 p.m.
       

    I understand that, if I wish to use a name on the ballot other than my First Name (or a common derivative of it) or my First Name (or a common derivative of it) and Middle Initial, I must complete and submit the Ballot Name Affidavit by the applicable deadline:

    • For candidates filing with a Qualified Political Party: January 8, 2026 at 5:00 p.m.
    • For candidates filing with a Registered Political Party: April 27, 2026 at 5:00 p.m.
    • For candidates filing without a party (unaffiliated): June 15, 2026 at 5:00 p.m.


    I understand that I may withdraw my candidacy by filing a written affidavit with the Salt Lake County Election Division any time up to April 24, 2026 for the Primary Election and September 4, 2026 for the General Election and have my name removed from the ballot.

     

    I am aware of the Statewide Electronic Voter Information Website Program and am aware of its submission deadline of 5:00 pm on May 8, 2026, for the Primary Election and 5:00 pm on September 4, 2026 for the General Election and I have received a copy of Section 20A-7-801 regarding the Statewide Electronic Voter Information Website Program.
      

    I have received a copy of the Campaign Finance Disclosure laws.
      

    I agree to file all Campaign Financial Disclosure Reports by the required dates, and I understand that failure to do so may result in my disqualification as a candidate for this office and removal of my name from the ballot.
      

    I understand that I will receive all financial disclosure notices by email.
      

    I have received a copy of the Pledge of Fair Campaign Practices, and I understand that signing the pledge is voluntary.

     

    I understand that I must complete the Alternate Contact Information Form if I wish for my residential address or phone number to be private.

  • I have submitted either a letter from the Utah Supreme Court affirming that I am an attorney in good standing, proof of my application with the Utah State Bar with an affidavit describing the status of my application, or an affidavit describing how I intend to comply with the requirements for office of county attorney described in 20A-9-201(3)(b)(i) prior to taking office.

  • I have provided evidence that I have either successfully met the standards and training requirements established for law enforcement officers under Title 53, Chapter 6, Part 2, Peace Officer Training and Certification Act, or that I met the waiver requirements in Section 53-6-206.

  • I have provided evidence that I have an active professional certification in good standing as either a certified public accountant, certified internal auditor, certified fraud examiner, certified management accountant, or certified information systems auditor.

  • I acknowledge that my name will not appear on the ballot.

  • I understand that all signature packets must be received by the County Clerk before the applicable deadline below:

    • For candidates filing with Qualified Political Parties, 14 calendar days before the party's convention.
    • For candidates filing with Registered Political Parties, March 31, 2026 at 5:00 p.m.
    • For candidates filing without a party (unaffiliated), June 15, 2026 at 5:00 p.m.
  • Pledge of Fair Campaign Practices

  • There are basic principles of decency, honesty, and fair play which every candidate for public office in the State of Utah has a moral obligation to observe and uphold, in order that, after vigorously contested but fairly conducted campaigns, our citizens may exercise their right to a free election, and that the will of the people may be fully and clearly expressed on the issues. 

     
    I SHALL conduct my campaign openly and publicly, discussing the issues as I see them, presenting my record and policies with sincerity and frankness, and criticizing, without fear or favor, the record and policies of my opponents that I believe merit criticism
     
    I SHALL NOT use, nor shall I permit the use of, scurrilous attacks on any candidate or the candidate's immediate family. I shall not participate in, nor shall I permit the use of, defamation, libel, or slander against any candidate or the candidate's immediate family. I shall not participate in, nor shall I permit the use of, any other criticism of any candidate or the candidate's immediate family that I do not believe to be truthful, provable, and relevant to my campaign.
     
    I SHALL NOT use, nor shall I permit the use of, any practice that tends to corrupt or undermine our American system of free elections, or that hinders or prevents the free expression of the will of the voters, including practices intended to hinder or prevent any eligible person from registering to vote or voting.
     
    I SHALL NOT coerce election help or campaign contributions for myself or for any other candidate from my employees or volunteers
     
    I SHALL immediately and publicly repudiate support deriving from any individual or group which resorts, on behalf of my candidacy or in opposition to that of an opponent, to methods in violation of the letter or spirit of this pledge. I shall accept responsibility to take firm action against any subordinate who violates any provision of this pledge or the laws governing elections.
     
    I SHALL defend and uphold the right of every qualified American voter to full and equal participation in the electoral process.
     
    I, the undersigned, candidate for election to public office in the State of Utah, hereby voluntarily endorse, subscribe to, and solemnly pledge myself to conduct my campaign in accordance with the above principles and practices.

  • CAUTION:

  • Submitting this form is only the first step to declaring candidacy. 

    To complete the process, you must attest to and sign the forms in office during the filing period at the Salt Lake County Clerk Election Division office.

    If you have not done so, you also must complete your Conflict of Interest Disclosure form prior to signing in office.
